Seems pretty straightforward for in person. Fill out a form, make an appointment. Usually, the confirmation emails about the appointment will tell you what to bring specifically.

Thank you. I have the filled out form and appointment is made. The emails did not tell me specifically what to bring. I see several lists of required documentation for other instances, but not specifically for a replacement license.

I mean, they allow you to fill out the form and e-mail it to them. Then, they send you a link for payment. It doesn’t mention about submitting additional documents. If you’re worried you won’t have something. Then, I’d go that route. The link I provided, you just change the tab towards the bottom from in-person to e-mail, for that info.

I'll probably try that, thank you for your help. I don't see the DMV not requiring more proof of ID, especially since i dont have the license number on hand, but If it's that easy then ill be pleasantly surprised :)

To request a duplicate driver’s license by email: Complete a request for a Connecticut driver’s license/identification card by mail (Form B-350) Email completed form(s) to: DMV.CIU@ct.gov To pay for your request, a link will be emailed to you within two business days. If there are any issues with your request, we will notify you via email. Your duplicate credential will be mailed directly to the address on file with the DMV and you will receive it within 20 days.
