Replace the fixture with an led version since you have it uninstalled. No more bulb problems.

Looks like an incandescent Lumiline. An LED retrofit is in order.

Oh wow, thanks for finding the type. Retrofit seems like the best/only way forward, that bulb seems a bit too antiquated to even source.

That seems like the path forward, will probably go down that route.

It could be a T-5 lamp. The lamps are measured in 1/8" increments. A T-8 lamp has a diameter of 1". A T-12 has a 1½" diameter, so the T-5 is 5/8".

But definitely get an LED replacement.
