Yes, it can be done. Years ago, we had our front concrete walkway leveled to address water flow also. They used a massive crow bar to lift the slabs, regraded the ground, added filler and replaced the concrete slabs. Not sure if this works with severely cracked slabs. Good Luck!

They can also drill holes in the slab and pump grout under it too lift it. It can work very well

Personally, I would hire someone. If they they messed up it would be cheaper than if you messed up, y'know?

I’m researching a local shop to do so… worried there won’t be much available if at all where I am.

I have leveled/moved sidewalks and a concrete patio using this method. Floor jack, shovel, and a new substrate. Dig a hole under the slab, insert floor jack to lift and apply new substrate to level. You can easily move a slab by inserting a few thick wall pvc pipes, 4", under the slab and roll them away.
