Pepito is the puertorrican (hispanic, in general?) version of Little Johnny. This joke likely translates very well into Spanish.

Yeah Pepito is the Hispanic (mostly Latin American) version of little Johnny. Clearly the joke is originally in Spanish. And yea it’s probably better in Spanish.
