see the joke here, fellow viewer is that 'Tony' sounds like 'toe-knee', which is what you get when you remove a mans shins, the part inbetween the toes and the knees

Ah, I see. Toe-knee. Toe. Knee. Because his shins have gone, so his feet, which have toes, are connected directly to his knees instead of indirectly via his shins. Hence, "Toe-Knee". I understand now, thank you.
