Asian pronunciation, L and R tend to both sound like R. So the joke is meant to be the guy who wanted his anus bleached, and due to his accent made it sound like he wanted to be fucked in the arse.

I get that, that doesn't make it a joke, now it makes sense bc it's from family guy, something that might be funny on a TV show, but taking a funny line from a TV show doesn't translate into a joke...
