A big part of the ACA reforms was that it set up a separate division at DOJ that just goes after Medicaid/Medicare fraud. It's paid for itself every year. A big part of why we still have issues is that the wealthiest man in congress, Senator Rick Scott (R-Fl), made his fortune bilking Medicare. As CEO his company was involved in an auto-bill scam that was, at the time, the largest Medicare fraud in US history, totaling over $ 1 billion. He pleaded the 5th nearly 100 times in the trial, pretending he had no idea how it could happen. He also opposed the ACA's fraud reforms.

Dan Meusser, a PA congressman reputed to be a resident of NY, also made his fortune by bilking Medicare via kickback schemes. Only fined $80,000. Was not forced to disgorge the ill-gotten gains thanks to the loving hands of his pal, then US Attorney for the Middle District of PA, Tom Marino.

No doubt why the Republicans put him in charge of the NRSC - if anyone knows how to defraud the government he should rise to the top. He took his job so seriously that he went to Italy to escape when they audited the committees finances.
