Pretty sure they meant that you always have a third option. Like if you'd rather die than take either option in the hypothetical. Most people interpret it as "which of these two options, you have to pick one" which is absolutely the point of the phrase. OP is saying you can get out of it by saying fuck it, pull the trigger.
I don't think they meant that they never realized that a gun to your head forces you to make a decision.

It's extremely common, and it's just an expression that means "if you had to choose right now, which of these two options would you pick?" Not "Answer this question or I'll murder you."
