How come AMD themselves made such a faux pas while their third party devs had adequate cooling? I kind of get it because the reference cards are always so generic and modestly designed where as vendors throw fans and exaggerated cooling fins everywhere.

In my experience, AMD reference cards are usually over-designed and highly tunable, whether you want to tune for speed or efficiency. They might screw up a cooler every now and again, but reference cards get aftermarket coolers made for them. The Tahiti reference board is a good example.
