On #1, do you happen to know if it is possible to play Spotify from iPhone->Cheapo SmartWatch->Bluetooth
Or is my 3-step idea not something that happens?
Like is it just iPhone->Bluetooth / OR / SmartWatch->Bluetooth
(I have zero experience with smart watches, so that might be a dumb question).

No because that was an example. You want the watch to be the source for the music only. You can wear it anywhere without worrying about a phone.
When I was in the US I had a data plan with my watch so I could stream music without a phone with the watch only.
Looking more into it you can use a Fitbit 5 add music to it and sync the Fitbit to your wireless earbud of choice.
