Good point. All three movies occur within a span of about a year and a half.

One thing that bothers me is that he never once called Mr. Miyagi “sensei”. Calling him “Mr. Miyagi” always seemed very disrespectful to me (even during Cobra Kai).

How is that disrespectful? Calling him Mr. Miyagi is his name, formally & respectfully. How were you brought up?

Because Miyagi WAS his sensei. Larusso trained under him. Yes, “Mr” is formal, but within the context of Miyagi being his mentor and practically like an uncle to him, “sensei” is more appropriate.
