Apocalypse Now's climax comes to mind, but also this scene from maybe an hour earlier:
"I told you not to stop. Now let's go."
I just wanted some mangoes!
Saving Private Ryan, when the dude isgetting stabbed really slowly.
Oskar Schindler talking about how he could have saved more
In Misery when she whacks his ankles
Final standoff in Reservoir Dogs
Dude getting his hand halfway cut off in Green Room
Nic Cage losing it in his undies in Mandy
The whole climax sequence of The Last of the Mohicans
Indigo Montoya gets his revenge in The Princess Bride
Bambi's mom getting shot / Mustafa getting killed
Don’t know if it’s quite what you mean, but Clint Eastwood’s Changeling (2008) comes to mind. All throughout the movie you’re thinking “This can’t be a true story”, but sadly it is.
Virgil (James Gandolfini) and Alabama's (Patricia Arquette) fight in the hotel room in True Romance.
That's a rough one.
McConaughey's final statement in A Time To Kill
That scene from Childern of men. You all know the one here is a link https://youtu.be/YBzWTIexszQ
Batman interrogating the Joker in TDK
Officer K in BR2049. When he finds out the truth about his memories and he freaks out.
When Casey Affleck has a breakdown in the police department in Manchester By The Sea.
The baby being born in Children of Men
John McClane walking across broken glass in his bare feet.
Ear sequence in Reservoir Dogs
The looks on the various faces as the reveal happens in Festen (1998)
The scene showing Andy breaking out of Shawshank. Incredible score, narration and visuals of him standing arms outstretched in the thunderstorm. I can't even imagine how great that looked watching it in theatres. I didn't get to see it until it hit On Demand PPV like 6 months later.
Setsuko eating mud while hallucinating them as food in Grave of the fireflies, then never waking up again.
Also, both Seita and Setsuko overlooking the present day city with Setsuko resting her head on Seita's lap.
There are two movies that I stopped watching halfway through because they were too intense. I was afraid they would trigger my depression.
One was Room (2015) -- not to be confused with The Room (2003). I'm told it has a happy ending but I never got that far.
Another was Mubound (2017). It's essentially the story of a lynching, preceded by torture. The torture scene was going on too long when I stopped watching.
They are both well-made films, and I might try again someday to watch them -- if I am feeling invincible.

The hotel room scene in Drive.
That was a wild one because that level of violence was not in the movie up until that point. Very jarring but in a good way.
For intensity, watch Nicolas Winding Refn's Pusher. It's nothing but pure, deep anxiety from beginning to end. Kinda like Uncut Gems (in a way), but way better and much more harrowing.
The T-Rex Paddock breakout scene from Jurassic Park. What made the scene so intense was that this was not a movie monster out for blood; it was an unpredictable wild animal just being an animal. It also happens to be the most fearsome predator to have ever walked the earth.
They easily could have had the Rex break out, go right after the kids immediately and start causing mayhem. Instead, the Rex looks and sniffs around, investigates things, acts curious, and slowly amps up the aggression once it starts realizing that there is prey inside the vehicles. Even before chomping on the lawyer, Rex cocks it’s head like a dog and looks at him trying to figure out what it is until it finally lunged.
Traditional monsters are fairly predictable. We know they are out to kill and harm people. They’re is little to no mystery with them. But an animal like a bear? You really have no idea what it will do.
