Most of the time I go to my healthcare provider and I have no idea what I will be charged until I see receive the EOB "Explanation Of Benefits" letter I receive weeks later from my insurance company. Negotiated rate? If the healthcare provider is willing to accept the "negotiated rate" for services every time then that should be their rate. We need a published rate across all healthcare providers that doesn't change based on whatever insurance provider a deal had been cut with.
