She's disabled so money isn't the issue, save for whether I'll have job when I come back, but that's neither here nor there.

If you qualify, FMLA will protect your employment for 12 weeks.

Unfortunately, yes, you have to be with your employer for 12 months. Talk to your HR, you may be to take unpaid leave.
