Play on either Blood & Broken Bones (2nd hardest difficulty) or Death March (Hardest) unless you prefer combat with no strategy involved anything below these two difficulties turns the game into a boring melee spam. You'll have to actually strategize for fights on the hardest settings you need to know your enemies strengths and weaknesses, apply oil to your blade, use potions, take advantage of bombs, and know what signs are best for the situation.

It's more of a problem with the publishers and greedy executives not the actual developers they just do what they are told to do and have to make a game with unreasonable deadlines which often leads to cutting corners and buggy releases.

Mass Effect 3 almost everybody hates it because of the endings but if you look past the last 15 minutes it's overall a great game.
