I believe you can play up the the most recent expansion for free, so to level 70 at least, maybe 80 now. Not sure on that.
You are locked out of the markets and free companies ("guilds" in other games) until you're a paying subscriber. I think those features enhance the game enough to be worth the $12/mo but that might not be true for everyone.
