Weapons not food, not homes, not shoes Not need, just feed the war cannibal animal I walk the corner to the rubble that used to be a library Line up to the mind cemetery now What we don't know keeps the contracts alive and movin' They don't gotta burn the books they just remove 'em While arms warehouses fill as quick as the cells Rally 'round the family, pockets full of shells

OP asked about songs that are metaphors, and Duality fits that mold. At no point is the word headache or migraine used in the song, unlike "Friday" being the song title and the most commonly used lyric in the song. You do not have to like Slipknot if that ain't your jam. But I feel the song fits OP's request, and while yes, I am old and so is Slipknot, they still produce new shit.

Check out The National and Manchester Orchestra (bands that have been around for a bit but still here).

Bon Iver. Taylor Swift and her work with The National and Bon Iver.

Metal very often has metaphors. Example, Duality by Slipknot is a song that mostly has to do with Corey experiencing horrible Migraines and writing a song around that

While more specific to a father the song, Fade In / Fade Out by Nothing More is amazing.

And for something a bit more visceral Hereafter by Architects. Also Death Is Not Defeat by Architects. The band was founded by twin brothers and one of them passed from skin cancer before the album those songs are on was written.
